On this page you will find a number of forms that we ask all parents/carers to complete and return to the school for the reasons stated.

Trips, visits and activities off site

Throughout a pupil’s time at Elemore Hall there will be many opportunities for them to take part in a wide range of off-site trips, visits and activities. Some of the events will take place during normal lesson time and be designed to complement or support the curriculum, others will be as part of regular evening activities, in addition there are occasional enrichment activities such as theatre trips that overlap the school day and into the evening. So that we do not need to seek written permission from parents/carers on every occasion, we prefer to use one form that covers most visits. It would be helpful if this form could be completed and returned to the school.

Free School Meals

The school does not charge any pupil for meals however it is still important for us to collect information regarding entitlement. It would be helpful if you could complete the form below and return it to the school.

Photograph agreement

As a school we take regular photographs of all pupils for our assessment records. We also like to take photographs and videos to use in displays around school, on the school website, occasionally in the press. We routinely record performances to show to others and the website is available to all. So that your child can be included in any photographs (or so that we can be careful not to include your child) please complete the form below and return it to the school.

Occasional Medication

It is our experience that sometimes pupils have simple medical complaints, such as headaches, that can be supported by over the counter remedies. By completing the form below you give the school permission to administer simple pain relief when necessary.

Prescribed Medication

Some of our pupils take prescribed medication – either for a long term condition or on a shorter term such as antibiotics. So that we can administer the medication safely in school we ask parents to complete and return the following form
